In late July our mail server was blocked in the network of MSN - Outlook, Hotmail, Live.com.

That causesd huge problems to us and our customers. We requested unblocking a lot of times and especially more information why the mail server was blocked and finally we got the needed information.
"MSN implemented a new tool for its mail services: Outlook.com Smart Network Data Services Deliverability to Outlook.com is based on your reputation. The Outlook.com Smart Network Data Services (SNDS) gives you the data you need to understand and improve your reputation at Outlook.com. But just looking at the data isn't enough! Maintaining a good reputation is a lot of work. You should use this data to keep your mailing lists clean and to monitor the IPs you control for unusual behavior. Reputation is always the responsibility of the sender. SNDS gives senders access to detailed data about individual IPs, and it also includes our Junk Email Reporting Program, which lets you receive reports when users junk your messages. Now you can view IP data and manage feedback loop settings from one convenient website."

That solution is intended to provide better functionality. However it seems that it is the reason for blocking mail servers all over the world. We continued requesting the mail server IP to be unblocked but it is still in their blocklist. Eventually we changed the IP with our second mail server IP and that was a workaround solution for our customers to be able to send emails. Furthermore we monitored the daily automatic reports by MSN indicating suspicous email accounts sending too many emails and we took the masures needed in order to avoind sending SPAM. However our second IP was blocked too on August 14. We immediately submitted requests for both IPs to be unblocked and now we wait for their response.

Addition and recommendation:

The only reason we can think of why MSN would blacklist us is because some of our customers use forwarded e-mail addresses to Microsoft accounts (Hotmail, MSN, Live or Outlook.com). If spam was sent to them through the forwarded e-mail and they mark it as such, the e-mail service provider sees us as the sender because we relayed the e-mail - we were the last mail server that touched it. To avoid this in the future, please, remove all forwarded e-mails to Microsoft customers.
